The Sourdough Project Questionnaire
Please fill out this questionnaire for a single starter. If you have additional starters, fill out separate questionnaires for each starter.
Once you complete the survey, you will receive a confirmation email with instructions for submitting a sample of your starter. Please fill out the questionnaire even if you do not plan to send a sample!
If you have questions about this project, you can contact us at firstname.lastname@example.org.
The Sourdough Project is collecting information to be used for research and your participation is voluntary. By completing any questionnaires, or submitting any samples of your starter, you indicate your consent to participate in the research. If you have any questions about your rights as a research participant, you can contact Deb Paxton (IRB Administrator) in the NCSU IRB office and reference IRB #10590 DAPaxton@ncsu.edu or (919)515-4514.
